Cancellation Terms & Agreement

By submitting this form, I acknowledge and agree to the following:

  1. Outstanding Balance

    I agree to pay any unpaid balance for services already rendered.

  2. Cancellation of Discounted Packages

    If I purchased a discounted multi-session package and cancel before completing all sessions, I understand that:

    • Completed sessions will be charged at the full single-session rate, and

    • I will be refunded the remaining balance, if any, after that adjustment.

  3. Cancellation Fees or Penalties

    If applicable, I understand my cancellation may be subject to an early termination fee, consistent with the membership or package I enrolled in.

  4. Notice Period I understand that cancellations must be submitted at least 48 hours before my next session to avoid additional charges. If submitted later, my final charge will apply and services will remain available through the end of the paid period.

  5. Membership Rate Protection

    I understand that by cancelling, I will forfeit my current membership rate. If I choose to return in the future, I will be subject to the current pricing at that time.

Verbal Cancellations Not Accepted

I understand that verbal requests, social media messages, or texts do not constitute an official cancellation. Only this form triggers the cancellation process.

  1. Optional Re-Engagement

    I understand I may be contacted by a team member to confirm my request or discuss alternative options before finalizing cancellation.
